Sarcoidosis: changing clinical manifestation in Japan.

چکیده
An increase in the number of patients with sarcoidosis in Japan has been recognized noticed in Japan since the 1960's. Sarcoidosis is a systemic disorder, the lesion of which is characterized as an epithelioid cell granuloma formation stimulated by unknown agents. In Japan, the majority of the patients show thoracic involvement which can easily be detected by annual check-up even when they are asymptomatic. The increase in the numbers became prominant during the 1970's and 1980's. However, as half of the patients were asymptomatic, and 75%of cases recovered, sarcoidosis was not clinically recognized as a serious disease during that period. However, in the 1990's, a substantial numberof these patients developed serious respiratory failure due to pulmonary fibrosis. The reason is that in 5%of the cases who were detected during the 1960's and 1970's the disease progressed within 10-15 years after the onset/detection (1 ). They could be justly evaluated as recipients for lung transplantation. Current issues to be solved are as follows: 1) What are the causative agent(s), 2) Whatpathophysiological processes cause epithelioid cell formation. From the histopathological similarity of the lesions between sarcoidosis and tuberculosis, sar-

99mTc-MIBI Washout Rate to Evaluate the Effects of Steroid Therapy in Cardiac Sarcoidosis
Objective: We sought to determine the usefulness of the 99mTc-MIBI (MIBI) washout rate for the evaluation of steroid therapy in cardiac sarcoidosis (CS). Methods: Eleven CS patients underwent MIBI myocardial SPECT both before and 6 months after initiating steroid therapy. The washout rate (WOR) of MIBI was calculated using early and delayed polar map images. The washout score (WOS) of MIBI was ...
متن کاملAn Isolated Mass in the Palm, Starting Manifestation of Sarcoidosis
Sarcoidosis is a systemic disease that is characterized with noncaseating granulomatous nodules which present in multiple organs specially lungs (90%) . Incidence of masses due to Sarcoidosis in upper extremity is low and most cases present in association with involvement of pulmonary hilary lymph nodes. In this article we present a rare case of Sarcoidosis which presented as a single soft ti...
متن کاملUsefulness of three-phase bone scintigraphy and SPECT/CT for the diagnosis of bone lesions of systemic sarcoidosis
We report a three-phase bone scintigraphy for the diagnosis of a peripheral bone lesion caused by systemic sarcoidosis. A 32-year-old man with suspected osteomyelitis of the right forefinger underwent three-phase bone scintigraphy with Tc-99m hydroxymethylene diphosphonate (HMDP) and single-photon emission computed tomography/computed tomography (SPECT/CT). The lesion was rich in blood flow acc...
متن کاملThe WASOG Sarcoidosis Organ Assessment Instrument: An update of a previous clinical tool.
INTRODUCTION A Case Control Etiology of Sarcoidosis Study (ACCESS) sarcoidosis organ assessment instrument has been used for more than a decade to establish uniform standards for the probability of sarcoidosis organ involvement. The ACCESS instrument has become increasingly outdated as new technologies have been developed. Furthermore, the ACCESS instrument failed to address all possible organs...
متن کاملFatal renal failure as the first manifestation of sarcoidosis diagnosed on necropsy in a young man: a case report.
Renal involvement as the first manifestation of sarcoidosis is rare and has never been reported in India.
